Before registering, please make sure you live within our Practice Are by entering your postcode below.

To be eligible to register at the practice, you must live within the practice catchment area.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.

New Patient Registration

Please fill in the below electronic form.  Please note you will be asked to upload ID.   Please note we can only register children if a parent or guardian is registered with the Practice or is registering at the same time.

If you have recently had a baby and you wish to register them at the surgery please use the New Baby Registration Form which is under Registrations

Safe Surgery Statement

In the UK, every individual is entitled to receive free care from a General Practitioner (GP), regardless of whether they can provide proof of identification or address. Registration is still possible even without these documents, and immigration status or nationality are not barriers here at Hartland Way Surgery. Our reception staff at Hartland Way Surgery do not request immigration documents and we will not disclose personal information to the Home Office unless there is involvement in serious criminal activity.
